Armplexus-Parese nach Spritzenabszeß des M. deltoideus

Wir berichten über einen 76jährigen Patienten der nach intramuskulärer Injektion von Diclofenac in den M. deltoideus bei bestehender Rippenserienfraktur ein Pleuraempyem einen Spritzenabszeß des M. deltoideus und eine Armplexus-Parese entwickelte. Im Abstrich konnte Staph. aureus nachgewiesen werden. Nach Spaltung des Abszesses Drainage des Pleuraempyems und systemischer Antibiose bildeten sich das Pleuraempyem der Oberarmabszeß nicht jedoch die untere Armplexus-Parese zurück. Als typische Komplikationen von intramuskulären Injektionen des M. deltoideus werden in der Literatur Kontrakturen Kalzifikationen und Spontanrupturen des M. deltoideus angeführt. Infektbedingte Armplexus-Paresen sind selten und nur in einigen wenigen Kasuistiken meist als Folge einer septischen Arthritis des Schultergelenkes beschrieben. Unseres Wissens ist der von uns vorgestellte Fall der erste einer Armplexus-Parese als Folge eines Spritzenabszesses. Intramuskuläre Injektionen in den M. deltoideus sind kontraindiziert und sollten ausschließlich Schutzimpfungen vorbehalten bleiben. Abstract Brachial plexus palsy following abscess after intramuscular injection in the deltoid muscle: We report on a case of a 76-year-old man who developed pleural empyema an abscess of the deltoid muscle and brachial plexus palsy after an intramuscular injection of diclofenac into the deltoid muscle. The patient received this injection due to persisting pain after serial fracture of the ribs. The culture was positive for Staph. aureus. Whereas the patient recovered from pleural empyema and abscess after surgical intervention and systemic antibiotics the brachial plexus palsy showed no improvement. Typical complications after intramuscular injections in the deltoid muscleare abscess contractures calcification and spontaneous rupture of the deltoid muscle. Brachial plexus palsy associated with an infection is a rare complication for example after septic arthritis of the shoulder joint and only few case reports exist. To our knowledge our case is the first one of brachial plexus palsy associated with an abscess after intramuscular injection. Intramuscular injections in the deltoid muscle are contraindicated and should only be performed in protective immunization.
