Behandlung und Rehabilitation nach Wirbelfrakturen

Mutschler) Homburg/Saar Zusammenfassung: Prädilektionsstellen für Wirbelbrüche sind die Kyphose- und Lordosescheitel der Wirbelsäule häufig der lumbodorsale Übergang. Die aktuelle Einteilung der Frakturformen nach der Pathomechanik und moderne operative Verfahren erlauben heute eine stringente Indikation. Die konservative Behandlung der Mehrzahl aller Wirbelfrakturen im lumbalen und thorakolumbalen Abschnitt ist unverändert frühfunktionell ausgerichtet. Operationsindikationen bestehen bei Verletzungen vom Typ B und Typ C bei manchen Verletzungen des Typ A sowie bei Beteiligung nervaler Strukturen. Unter biomechanischen Aspekten sind auch valide Rehabilitationskonzepte und Zeitvorgaben für die einzelnen Rehabilitationsschritte bei konservativ oder operativ behandelten thorakolumbalen und lumbalen Wirbelfrakturen gegeben. Treatment and rehabilitation of fractures of the thoracolumbar or lumbar vertebral column: The sites of predilection for vertebral fractures are the vertices of kyphosis and lordosis of the spinal column often the lumbodorsal junction. The current classification of forms of fractures according to the pathomechanic and modern operative procedure permit a stringent indication today. The conservative treatment of the majority of all vertebral fractures in the lumbar and thoraco-lumbar region is still primary function-oriented. Operative indications exist in injuries of Type B and Type C to a limited extent also in Type A and when nerve structures are affected. From the point of view of biomechanics valid rehabilitation concepts and time allowances also exist for the individual rehabilitation steps in conservative or operative therapy for thoraco-lumbar and lumbar vertebral fractures.
