Differentialdiagnostik der Angst nach ICD- 10

Abstract: Wolfgang Schneider und Paul L. Janssen Universität Rostock Nervenklinik Abteilung Psychosomatik und Psychotherapie Westfälische Klinik für Psychiatrie Dortmund Zusammenfassung Im Beitrag werden die Prinzipien der operationalen Diagnosensysteme wie DSM-III-R bzw. DSM-IV und ICD-10 aufgezeigt. Auf diesem Hintergrund werden dann die Klassifikation der Angststörungen sowie die wichtigsten für jede diagnostische Klasse definierten diagnostischen Kriterien dargestellt. Die Forschungskriterienstudie im Bereich der deutschen Psychosomatik/Psychotherapie hat für die Angststörungen eine gute Interraterreliabilität gezeigt die als ein wichtiges Qualitätsmerkmal diagnostischer Modelle anzusehen ist. Abschließend werden grundlegende Mängel der ICD- 10 aus der Perspektive der psychodynamisch orientierten Diagnostik diskutiert zu denen die mangelnde Konzeptualisierung von Krankheitsbildern sowie die geringfügige Aussagekraft für eine differentielle Therapieindikation gehören da wichtige indikationsleitende Merkmale wie z. B. die Persönlichkeitsstruktur charakteristische Konfliktbereiche die Lerngeschichte und die Krankheitsverarbeitung vernachlässigt werden. Abstract The Classification of the Anxiety Disorders according to ICD-10 The principles of operational psychiatric diagnostic systems like DSM-III-R DSM-IV and ACD-10 are shown at the beginning of the paper. On this background the classification of the anxiety disorders in ICD- 10 and the most important diagnostic guidelines and criteria are explained. The "diagnostic criteria for research" study in Germany in the field of psychosomatics and psychotherapy has shown good interrater reliability which is an important characteristic of the quality of the diagnosis. At last essential shortcommings of ICD-10 from the perspective of psychosomatics/psychotherapy are discussed. For example the disease conceptualization and the low treatment validity are missing because important diagnostic characteristics like personality structure the specific conflicts of the patient characteristics of learning and the coping mechanism are missing in ICD-10 and DSM.
