Probleme der Lendenwirbelsäule aus der Sicht des klinischen Orthopäden

Abstract: Dr. med. W. Dürrfeld Arzt für Orthopädie Caspar-Heinrich-Klinik Bad Driburg Zusammenfassung Im Laufe eines Lebens werden viele Menschen von Kreuzschmerzen geplagt. Viele Krankheitsbilder können sich dahinter verstecken. Zur Diagnosefindung und gezielten Therapie sind die genauen Kenntnisse der anatomischen Gegebenheiten der Bewegungssegmente die umfassende Anamnese Inspektion Palpation sowie die sorgfältige Erhebung des orthopädischen Status einschließlich der Chirodiagnostik und der neurologischen ggf. auch der röntgenologischen und laborchemischen Untersuchung unter Ausschluß von Störfeldern von besonderer Bedeutung. Differentialdiagnostisch sind insbesondere Lumbalsyndrome (LWS-Syndrom Lumbalgie Lumbago Facetten-Syndrom) Lumboischialgie und Bandscheibenprolaps abzugrenzen. Störungen besonderer Art zeigen sich häufig bei Spinalstenose Spondylolisthesis Lumbalskoliose Morbus Baastrup lumbosakralem Übergangswirbel Blockierungen im Lumbosakralbereich entzündlichen Wirbelsäulenerkrankungen Wirbelsäulentumoren Osteoporose und Morbus Bechterew. Sinnvolle Behandlungsmöglichkeiten sollen aufgezeigt werden. Summary In course of life many people suffer from lumbago and many symptomatologies may hide behind such pains. For diagnosis and purposeful therapy the exact knowledge of the anatomic conditions of the motorial segments the extensive anamnesis inspection palpation as well as carefully obtained orthopaedic status inclusive of the chirodiagnosis and the neurological eventually also the röntgenoscopical and chemical laboratory investigation with exclusion of disturbing fields are especially important. Especially lumbar syndromes (syndrome of the lumbar vertebral column lumbalgia lumbago facette syndrome) lumboischialgia and prolapsus of intervertebral disks have to be clarified by differential diagnosis. Special kinds of disorders are often found with spinal stenosis spondylolisthesis lumbar scoliosis lumbosacral transitional vertebra Baastrup's disease blockage in the lumbosacral region inflammatory diseases of the vertebral column spinal tumours osteoporosis and Bechterew's disease. Purposeful possibilities of treatment shall be shown.
