Antiretrovirale Therapie und Pneumozystose-Prophylaxe bei HIV-Infektion

Abstract: Dr. med. J. R. Bogner Medizinische Poliklinik München Zusammenfassung Zidovudin (AZT; Handelsname: Retrovir(r)) ist das bislang einzige in Deutschland zugelassene Medikament zur antiretroviralen Therapie der HIV-Infektion. Die Effektivität wurde von 1986 bis 1991 in zahlreichen klinischen Studien belegt. Eine Dosierung von 500 mg täglich gilt demnach als effektiv für die Progressionsverzögerung und Verminderung opportunistischer Infektionen - auch bei asymptomatischen Patienten mit einer meßbaren Verminderung der Helferlymphozyten. Für Patienten mit weniger als 250 Helferlymphozyten/l wird darüber hinaus eine Pentamidin-Aerosol-Prophylaxe empfohlen. Hierdurch wird die Wahrscheinlichkeit einer Pneumocystis-carinii-Pneumonie entscheidend verringert. Neuere Therapieansätze bestehen in der Entwicklung und klinischen Prüfung von Hemmstoffen der viralen Polymerasen und Proteasen sowie spezifischer Immuntherapien. Antiretroviral Therapy of HIV-infection and Prophylaxis of Pneumocystis Carinii Pneumonia: Up to now zidovudine (AZT; Retrovir(r)) is the only remedy having obtained health registration in Germany for antiretroviral therapy of HIV-infection. Its efficacy has been shown in various clinical trials from 1986 to 1991. A daily dosage of 500 mg reduces the probability of progression to symptomatic disease in asymptomatic patients and lowers the frequency of opportunistic infections in AIDS patients. Prophylaxis for pneumocystis carinii pneumonia with inhaled pentamidine reduces the incidence of pneumocystis carinii pneumonia in patients with helper lymphocyte counts below 250/l. The investigation of new drugs covers additional compounds inhibiting viral polymerases and proteases as well as new approaches to an active immunotherapy. ab
