Die geläufigen Brustdrüsenerkrankungen aus der Sicht des Pathologen

Prechtel Pathologie Starnberg Starnberg Zusammenfassung In der Spezifizierung des bioptischen Untersuchungsgutes der weiblichen Brustdrüse erscheinen drei Diagnosegruppen in überwältigender Mehrheit: Fibroadenome (DD: Phylloides-Tumor Hamartom Lipom) als meist abgerundete Tumoren mit einem Altersgipfel in der 4. Dekade mastopathische Veränderungen mit einem komplexen morphologischen Muster und einer Altershäufung in der klimakterischen Phase sowie Karzinome in meist sternförmiger Kontur und mit einem nicht bereinigten Altersgipfel um das 60. Lebensjahr bei allerdings breiter Streuung. Bei der Differentialdiagnose sollten in einem Gedankengang das Häufige das charakteristische Alter und die Kontur der Verschattungen im Mammogramm Berücksichtigung finden um möglichst gezielt diagnostische Methoden anzuwenden. Bei Einsatz der Tripeldiagnostik kann die Indikation zum exploratorischen chirurgischen Eingriff mit nachfolgender Histologie eingeengt werden. Summary In the specification of the specimens obtained by biopsy of the female mammae three groups of diagnoses appear as a great majority: fibroadenomata (differential diagnosis: phylloides tumour hamartoma lipoma) as mostly rounded tumours with a climax as to age in the 4th decade mastopathic changes with a complex morphological pattern and a frequency as to age in the climacteric phase as well as carcinomata in mostly stellar contour and with an over-all climax around the 60th year of age however with a wide deviation. In the differential diagnosis the frequency the characteristic age and the contour of the shadow in the mammogram should be considered in one train of ideas in order to avail oneself as purposefully as possible of diagnostic methods. Using the triple diagnosis the indication for an exploratory surgical operation with subsequent histological examination may be limited. wt
