Eingeschränkte Radikalität beim Rektumkarzinom Eine sinnvolle organerhaltende Behandlungsalternative mit kurativem Ergebnis?

Abstract: Prof. Dr. med. Ch. Herfarth Chirurgische Universitätsklinik Heidelberg Zusammenfassung Die operative Versorgung ist derzeit die einzige Therapie des Rektumkarzinoms die dem Patienten eine Chance auf Heilung bietet. Für das mittlere und untere Drittel des Rektums stehen eine ganze Reihe von Operationen zur Verfügung wie die abdomino-sakrale Rektumamputation die anteriore Rektumresektion und die lokale Exzision. Die Auswahl des geeigneten Verfahrens ist von der Invasionstiefe des Tumors seiner Erreichbarkeit und Größe und von der Anwesenheit perirektaler Lymphknoten abhängig. Zusätzlich muß der Differenzierungsgrad des Tumors Berücksichtigung finden. Während die Koloskopie die Höhe und Ausdehnung des Tumors intraluminal bestimmt gibt uns die Anwendung der Endosonographie Auskunft über Invasionstiefe und Lymphknotenmitbeteiligung. Nur durch strengste Selektion geeigneter Patienten unter kontrollierten Studienbedingungen erscheint uns die Anwendung von Verfahren mit eingeschränkter Radikalität angezeigt. Limited Radicality in Rectal Carcinoma. A Useful Organ-consenring Therapeutical Alternative with Curative Result? Surgery at present is the only treatment of rectal carcinoma offering the patient a chance of healing. A variety of surgical procedures is available for the middle and lower third of the rectum such as abdominosacral proctectomy anterior proctectomy and local excision. The choice of the appropriate procedure depends on the invasive depth of the tumour its accessibility and size and on the presence of perirectal lymph nodes. In addition the degree of differentiation of the tumour requires consideration. While coloscopy determines the height and extent of the tumour intraluminally the use of endosonography informs us on the depth of invasion and involvement of lymph nodes. It is only by strictest selection of suitable patients under the conditions of a controlled study that we deem the use of procedures with a limited radicality indicated. ab
