Ergebnisse einer kontrollierten Verlaufsuntersuchung über den Einfluß physikalischer Kombinationstherapien auf die Reaktivierung postoperativer Innervations- und Kraftdefizite bei Knieinstabilitäten

Mucha Abteilung Physikalische Medizin und Rehabilitation-Zentrum Radiologie-der Medizinischen Hochschule Hannover Hannover Zusammenfassung Im Gruppenvergleich mit jeweils 20 Patienten nach Operation einer anterior-medialen Rotationsinstabilität wurde der Einfluß einer Kombinationstherapie im Vergleich zur übungstherapeutischen Monotherapie auf die Reaktivierung der Innervation und Kraft des M. quadriceps femoris geprüft. Die Ergebnisse wurden für einen 10wöchigen Behandlungs- und Beobachtungszeitraum nach Beendigung der Gipsimmobilisation im Verlauf ausgewertet und zeigen eine signifikante Regenerationsbeschleunigung der postoperativen gravierenden Innervations- und Kraftdefizite in der Gruppe die mit der Kombinationstherapie versorgt wurde. Zudem ließ sich auch ein Innervationsdefizit des M. biceps femoris nachweisen das jedoch gegenüber demjenigen vom M. quadriceps femoris deutlich geringer ist. Innervations- und Kraftleistungen zeigen deutlich geschlechtsspezifische Unterschiede. Die Bedeutung eines abgestimmten Ausgleiches dieser motorischen Dysbalance für das Rehabilitationsprocedere und mögliche Stabilitätsergebnis wird diskutiert. Summary Results of a controlled study of the course about the influence of physical combined therapies an the reactivation of post-operative deficits of innervation and strength an knee - instabilities Comparing two groups of 20 patients each after the operation of an anterior- medial rotatory instability it was tested the influence of a combined therapy in comparison to an exercise mono-therapy on the reactivation of innervation and strength of the m. quadriceps femoris. The results were analyzed during a course of a 10 weeks' of treatment and observation period after the end of plaster cast immobilization. They demonstrated a significantly accelerated regeneration of the post - operative aggravating deficits of innervation and strength in the group performing the combined therapy. In addition to that it could also be proved a deficit of innervation of the m. biceps femoris. However this deficit appeared significantly smaller compared to the deficit in case of the m. quadriceps femoris. The performances of innervation and strength show significantly sexualspecific differences. The meaning of a harmonized equalization of this motor dysbalance is discussed with regard to both the organization of rehabilitation and possible results of the stability. hl
