EMG-Biofeedback-Training bei Patienten mit Hemiparese und Fallfuß1

Abstract: Aus der Abteilung für Physiotherapie (Leiter: Doz. Dr. sc. Med. A. Lange) der Medizinischen Akademie "Carl Gustav Carus" Dresden Klinik für Innere Medizin (Direktor: Prof. Dr. sc. med. K.-U. SCHENTKE) 1 Herrn Prof. em. Dr. sc. med. H. EDEL zum 75. Geburtstag gewidmet Zusammenfassung Die Ergebnisse eigener Untersuchungen an 4 Pat. mit Hemiparese und Fallfuß erlauben in Übereinstimmung mit anderen Autoren die Feststellung daß das EMG-Feedback-Training (EMG-FB) der Fußheber eine Bereicherung des Behandlungsspektrums im Rahmen der Rehabilitation von Patienten mit Hemiparese darstellt. In einer weiteren Studie an 6 Pat. galt es durch quantitative Erfassung des integrierten EMG mittels Oberflächenableitung vom M. tibialis anterior der gelähmten Seite herauszufinden ob die Effektivität des EMG-FB der Fußheber durch eine spastiksenkende Vorbehandlung des M. triceps surae mit Reizstrom oder Kryotherapie gesteigert werden kann. Aufgrund der Ergebnisse erscheint eine solche spastiksenkende Vorbehandlung vor der eigentlichen Feedback-Therapie sinnvoll zu sein ohne daß sich bereits eine eindeutige Aussage zu Gunsten einer der beiden spastiksenkenden Maßnahmen treffen läßt. Summary The results of own examinations an 4 patients with hemiparesis and drop foot allow in correspondence with other authors the establishment that the EMG feedback training of the levators of the food represents an enrichment of the spectre of treatment in the framework of the rehabilitation of patients with hemiparesis. In another study an 6 patients the task was to find out by quantitative Statement of the integrated EMG by means of surface lead from the anterior tibial muscle of the paralysed side whether or not the effectiveness of the EMG feedback training of the levators of the foot maybe enlarged by a spacicity-reducing pretreatment of the triceps surae muscle with the help of stimulation current or cryotherapy. On account of the results of such a spasticity-reducing pretreatment seems to be reasonable before the actual feedback therapy without already making an unequivocal statment in favour of one of the two-spasticity-reducing measures. ___MH
