Possibilities for prophylaxis and rehabilitation of patients after myocardial infarction at sea-side environment |

JARAMLAKOV National Center of Physical Therapy and Rehabilitation Sofia Bulgaria Short-thermal rehabilitation (30 days course) in patients with myocardial infarction have been carried out during different seasons of the year in health maritime resorts of Bulgaria. The conducted ergometric evaluation of the physical working capacity (PWC) was a real precondition for working out rehabilitation programmes including two different motive regimens : training and spare-training for the patients with lower functional capacity. The compositive parts of the programmes are as follows : therapeutic exercises with increased emotional effect and greater physical loading walking tours including "positive" and "negative" work dosed by duration and physical activity with or without helio-exposition. In order to evaluate the thermoenergetic characteristics of the environment two mathematical methods were applied : "the water vapour pressure at the bodysurface" and "the radiation-effective-equivalent temperature" . The results obtained show significant augmentation of the physical working capacity in 78 7 % of the treated patients and worsening in 21 3 %. The latter were in significant correlation with the lower coronary reserve of the patients and with the thermoenergetic characteristics of the environment. The favourable results demonstrate the importance of the applied rehabilitation programme carried out during periods when the thermoregulation of the organism is not burdened. The conclusions obtained proving the necessity of seasonal indication and differentiated approach in rehabilitation on patients after myocardial infarction. Some architectural barriers and transportation problems for the patients with disability tendency are under consideration in the report. wt
