Abstract: With the help of four theses, the situation of the German health service at the turn of the millennium is discussed critically. The German welfare state is about to become a health state with centralized planning and intervention competence. The results must not be underestimated, because on the other hand the trend of individualization and pluralization towards health services in society is increasing. The requirements of the state with disgusting intervention in administration and threats of sanctions like the rigidity of corporations with administrative over-regulation and the reduction of medical services are examples of a obsolete hierarchy of the German welfare state. A new understanding of health as quality of life has to be developed.
