Wie gut sind Deutschlands Karotischirurgen? Bilanz nach 15 000 Eingriffen

Giovanni Torsello Chefarzt der Klinik für Gefäßchirurgie St. Franziskus-Hospital Münster Zusammenfassung Um die Behandlungsqualität der chirurgischen Therapie von Patienten mit stenosierenden Karotisprozessen auch im Sinne der Gesetzgebung sicherzustellen hat die Deutsche Gesellschaft für Gefäßchirurgie (DGG) eine patientenbegleitende Erfassung von prä- intra- und postoperativen Daten von Karotisrekonstruktionen etabliert. Mittlerweile liegen Daten aus 158 Zentren über 15 116 Eingriffe vor. In Relation zur Literatur war die Frequenz von schweren postoperativen neurologischen Defiziten vergleichsweise gering Nur bei 1 9% der Fälle trat ein schweres neurologisches Defizit auf. Die perioperative Letalität lag bei 1%. Statistisch gesehen zeigte sich daß das Morbiditäts- und Letalitätsrisiko vom Risikoprofil des Patienten und von Form Art und morphologischer Beschaffenheit des Karotisverschlußprozesses abhängig war. A Challenge and an Opportunity With the aim of assuring the quality of surgical treatment of carotid stenoses - also in conformity with legal requirements - the German Vascular Society (DGG) has established the recording of surgical data of patients Prior to during and following carotid reconstructive procedures. Since its inception the data from 158 Centers covering 15 116 interventions are now available. In comparison with data in the literature the rate of severe Post-operative neurological deficits was found to be only 1.9% and the mortality rate 1%. Statistical analysis has shown that the risks of morbidity and mortality depend an the patient's risk Profile and an the nature and morphology of the carotid lesion.
