The role of cytokines and growth factors in cartilage destruction in osteoarthritis and rheumatoid arthritis

Rheumatology Dept. of Rheumatology University Hospital Nijmegen Die Rolle von Zytokinen und Wachstumsfaktoren bei der Knorpeldestruktion bei Arthrose und rheumatoider Arthritis Summary Cartilage destruction in arthritis and osteoarthritis is linked to aberrant cytokine and growth factor expression in the affected tissues. It becomes clear that the balance of protective and destructive cytokines is more important for the net destruction than the absolute levels of destructive mediators. IL-1 is a key destructive mediator in arthritis and probably also in osteoarthritis. Production of the cartilage destructive enzyme stromelysin is linked to IL-1. In osteoarthritis excessive formation of the growth factor TGF-Beta may contribute to cartilage lesions and osteophyte formation in particular. Therapy should be aimed at neutralization of IL-1 and stimulation of safe anabolic growth factors for the articular cartilage such as IGF-1 and the novel bone and cartilage derived morphogenetic proteins. Zusammenfassung Die Knorpeldestruktion bei der Arthritis und der Arthrose ist an eine überschießende Expression von Zytokinen und Wachstumsfaktoren im betroffenen Gewebe gebunden. Es wird offenkundig daß das Gleichgewicht zwischen protektiven und destruktiven Zytokinen wichtiger für die resultierende Zerstörung ist als die absoluten Spiegel destruktiver Mediatoren. IL-1 ist ein destruktiver Schlüssel-Mediator bei der Arthritis und möglicherweise auch bei der Arthrose. Die Bildung des knorpelzerstörenden Enzyms Stromelysin ist an IL-1 gebunden. Bei der Arthrose kann die überschießende Bildung des Wachstumsfaktors TGF-Beta zur Knorpelzerstörung und besonders zur Osteophytenbildung beitragen. Die Therapie sollte auf die Neutralisation von IL-1 und die Stimulation sicherer anaboler Wachstumsfaktoren für den Gelenkknorpel gerichtet sein wie beispielsweise IGF-1 ferner auf neue dem Knochen und Knorpel entstammende morphogene Proteine.
