Palliative Strahlentherapie Rolle im interdisziplinären Behandlungskonzept bei fortgeschrittenem Tumorleiden

Abstract: Priv.-Doz. Dr. med. M. H. Seegenschmiedt Leitender Arzt der Klinik für Radioonkologie Strahlentherapie und Nuklearmedizin Alfried Krupp von Bohlen und Halbach Krankenhaus Essen. Viele tumorbedingten Symptome können mit Strahlentherapie kausal wirksam effektiv und schonend behandelt werden. Das klinische Ansprechen beträgt 80% und ist unabhängig von der Histologie des Tumors. Eine sorgfältige Indikationsstellung und genaue Lokalisation der Symptomursache sowie exakte Planung und Durchführung der Strahlentherapie sind Bedingung. Mit einer individuellen Anpassung der Einzel- und Gesamtdosis des Zielvolumens und dem Einsatz moderner Planungs- und Bestrahlungstechniken gelingt es oft innerhalb weniger Tage bis Wochen eine langanhaltende Symptombesserung ohne wesentliche Nebenwirkungen zu erzielen. Die palliative Strahlentherapie ist dabei Teil eines Gesamtbehandlungskonzeptes das interdisziplinär abgestimmt ist und eine optimale Lebensqualität für den Tumorpatienten anstrebt manchmal auch unter Verzicht auf eine Therapie. Abstract Management of patients with advanced tumours - the role of radiotherapy within an interdisciplinary concept: Many tumour symptoms can be treated effectively and gently with radiotherapy. The clinical response rate is 80% independent of the tumour histology. Careful definition of the indications and accurate localisation of the cause of the symptoms as well as precise planning and administration of radiotherapy are essential. With individual adjustment of the single and total dose the target volume and the use of modern planning and irradiation techniques it is often possible to achieve prolonged symptomatic improvement within a few days or weeks without appreciable side effects. Palliative radiotherapy forms part of an overall consensual interdisciplinary management concept with the aim of providing the best possible quality of life for the patient sometimes even by withholding extensive therapy.
