Abstract: Prof. Dr. med. Thure von Uexküll zum 90. Geburtstag gewidmet. Prof. Dr. med. W. Schüffel Abteilung für Psychosomatik Zentrum für Innere Medizin Klinikum der Philipps-Universität Marburg. Behandlung im Krankenhaus läuft zunehmend als Drehtürmedizin ab; sie weist Merkmale einer virtuellen Digitalwelt auf. Die durchschnittliche Aufenthaltszeit im internistischen Kompartment beträgt derzeit 6 3 Tage (Zentrum Innere Medizin Marburg; außer Psychosomatik). - Die Abteilung Psychosomatik die in das Zentrum Innere Medizin (ZIM) der Universität Marburg integriert ist hat seit 1976 ein Behandlungskonzept mit durchschnittlich 36 Tagen entwickelt. Der Patient wird in seiner Eigenverantwortung angesprochen d. h. gleichermaßen auf seine ihm unbewußten Widerstände wie seine Ressourcen. Er durchläuft hierbei unter qualitätssichernden Aspekten einen Prozeß des Umganges mit Affekten eines Containment neuartiger Schematabildung und des Empfindens bzw. Ergreifens von Handlungsmöglichkeiten. Die Konzepte der Stimmung (v. Uexküll) der Salutogenese werden herangezogen neuartige "Strukturen" (sog. Emergenzen) einschließlich neuartiger Körperwahrnehmungen (subjektive Anatomie) entstehen zu lassen. - Prozeßorientiert werden die Möglichkeiten eines Großklinikums mit seinen drei Hauptstrukturen Abteilungsbereich Stationsbereich interdisziplinärer Konsiliarbereich einbezogen. Es schließt sich die geplante Rückführung in die extramurale Psychosomatische Grundversorgung an. - Die Umsetzung des zugrundeliegenden systemischen Ansatzes stößt auf jeder Ebene auf neue Chancen (neuartige Sozialisation; z B. Anamnesegruppen) wie auf Hindernisse. Der Ansatz hilft die virtuelle Welt der Drehtürmedizin zugunsten einer realen Welt des Patienten zu überwinden. Sie wird sich mittel- und langfristig nicht nur als preiswerter erweisen. Sie hilft verlorene Sinnbezüge in die Behandlung im Krankenhaus zurückzuholen. Abstract The other hospital treatment - why 1 x 36 is not equal to 6 x 6: Treatment in hospital is becoming increasingly "revolving door" medicine. It has the characteristics of a "virtual" digital world. The period of stay in an internal medicine department is currently 6.3 days (Zentrum Innere Medizin [Internal Medicine Centre] Marburg; apart from the Psychosomatic Department). The Psychosomatic Department which is integrated into the Internal Medicine Centre of the University of Marburg has developed since 1976 a treatment concept lasting for an average of 36 days. The patient is addressed an the basis of his own responsibility i. e. his subconscious resistance and his own resources are dealt with equally. He undergoes a process under aspects of quality assurance concerning the handling of emotions containment formation of novel patterns of behaviour and of sensing and taking possible action. The concepts of mood (von Uexküll) and salutogenesis are invoked to allow the development of novel "structures" (known as emergents) including novel forms of body awareness (subjective anatomy). The possibilities presented by a large hospital with its three main structures - departments. wards and interdisciplinary consultation - are involved in the process. It is rounded off by the planned return to extramural basic psychosomatic care. The implementation of the underlying systemic concept impacts at every level with new opportunities (novel socialisation e. g. peer groups an medical history taking) and obstacles. This concept helps to overcome the virtual world of revolving door medicine in favour of the real world of the patient. It will not only prove to be less expensive in the medium and long-term it will also help to recover the lose sense of direction of treatment in hospital.
