Proximaler Tubulusschaden durch Fumarsäuretherapie der Psoriasis

Koch Zusammenfassung Unter der langjährigen Fumarsäuretherapie einer Psoriasis waren bei einer 38jährigen Frau Glukosurie Hypophosphatämie Proteinurie und Hypourikämie aufgetreten. Der tägliche Fumarsäureanteil betrug 0 94 g über fünf Jahre. Trotz Absetzung der Medikation persistierten auch nach einem halben Jahr noch die Symptome Proteinurie und Glukosurie während nur ein leichter Anstieg des Harnsäurespiegels zu verzeichnen war und Phosphat weiter p.o. supplementiert werden mußte. Ein persistierender proximaler Tubulusschaden unter Fumarsäuretherapie wurde bisher nicht beschrieben. Summary Long-term therapy (several years) with fumaric acid esters in a 38 yearold female psoriasis patient was associated with glucosuria hypophosphataemia proteinuria and hypouricaemia. The patient had been treated with daily fumaric acid dosages of 0.94 g for a period of 5 years. Proteinuria and glucosuria symptoms persisted even six months after the discontinuation of therapy and uric acid levels had increased only slightly. Peroral phosphate supplementation had to be continued. This is the first report in the literature of persistent proximal tubular damage on fumaric acid therapy.
