Abstract: Povacz P. et al.: A randomized prospective study of operative and non-operative treatment of injuries of the fibular collateral ligaments of the ankle. J. Bone Joint Surg. 80-A (1998) 345-351 Most injuries of the fibular collateral ligament of the ankle are treated non-operatively. Some argue that non-operative treatment is occasionally followed by chronic laxity of the ankle. As this laxity compromises a patient's return to athletic activity some argue for operative repair. This study sought to prospectively compare functional and radiographic results of operative versus nonoperative repair of injuries to the fibular collateral ligament of the ankle. This case controlled study included 146 patients diagnosed with acute ankle sprain consistent with a disruption of the fibular collateral ligament. The diagnosis of injury was established by physical examination and stress radiographs. Subjects were randomly assigned to either operative or non-operative treatment. Patients were excluded from this study if they had a history of ankle injury or instability. The surgical group underwent reanastomosis of the torn ligament within 72 hours of the injury. Post-operatively the ankle was placed in a below-knee plaster cast for six weeks. The non-operative group was given an ankle orthoses for a period of six weeks.
