Self-reported depression familial history of depression and fibromyalgia (FM) and psychological distress in patients with FM

Ackenheil Department of Psychiatry University Hospital Munich München Germany Summary The prevalence of FM in the general population is estimated at 2%. FM is among the three most common diagnoses in ambulatory adult rheumatology practice. To study the degree of depression the familial history of depression and FM as well as the psychological distress in our FM population we mailed a standardized questionnaire to 304 FM patients. The response rate was 33%. We found BDI scores higher than 21 in 27% of the patients indicating clinical relevant depression. The patients had high levels of global distress measured with the SCL-90-R as well as elevated scores in the subscales. Twenty three percent had a familial history of depression 46% a familial history of FM and 46% had been diagnosed with depression in the past. Zusammenfassung Die Prävalenz der FM wird in der Bevölkerung mit 2% angegeben. In der rheumatologischen Praxis gehört diese Erkrankung zu einer der 3 häufigsten Diagnosen. Wir haben 304 FM Patienten unserer Abteilung einen standardisierten Fragebogen geschickt um das Ausmass einer Depression eine familiäre Belastung mit Depression und Fibromyalgie sowie den Umfang einer möglichen psychischen Belastung zu bestimmen. Von den 100 Patienten die geantwortet haben wiesen 27% (> 21) erhöhte Werte im Beck Depressions Inventar auf die hinweisend auf eine klinisch relevante Depression sind. Bzgl. der psychischen Belastung gemessen mit dem SCL-90-R fanden sich erhöhte Skalenwerte sowohl bei der Gesamtbelastung als auch bei den Subskalen. 23% der Patienten hatten eine familiäre Belastung bzgl. einer Depression und 46% bzgl. einer Fibromyalgie.
