Grundlagen der Schmerztherapie mit Lokalanästhesie

Abstract: Edwin Klaus Zusammenfassung Aus historischer Sicht wurde der Grundstein für diese Therapie vor etwa 100 Jahren gelegt als es gelang Procain aus einem Derivat des Kokains zu synthetisieren. Aus der Idee der Anwendung dieses Lokalanästhetikums entstand in einer langen Reihe von Bemühungen die Neuraltherapie auch Heilanästhesie genannt. Mittlerweile hat sich hieraus die mit neueren Erkenntnissen verbundene therapeutische Lokalanästhesie (TLA) entwickelt. Die Notwendigkeit einer solchen Therapie leitet sich aus dem Bedürfnis ab nicht nur akute sondern auch chronische Schmerzen behandeln zu können. Die Bedeutung von chronisch ist ja eigentlich gleichzustellen mit Therapieresistenz. Gleichzeitig hat der Schmerz beim chronischen Geschehen seine Bedeutung als Warnfunktion verloren. Somit ist der chronische Schmerz zu einem eigenständigen Krankheitsbild geworden. Dies besagt jedoch nicht daß akute oder auch funktionelle Schmerzen durch diese Therapie nicht behandelt werden können. Abstract Concepts of pain therapy by local anesthesia The treatment of chronic pain consists of four basic concepts: Drugs (analgetic drugs TAD etc.) treatment by physicians (chiropraxis massage TENS etc.) injection with local anesthetics and autosuggestion. Necessary for diagnosis and treatment of chronical pain is the knowledge of pathophysiology and anatomy of nerves ligaments muscles and the sympathetic nervous system. Diagnosis of chronical pain rarely includes roentgenograms or other technical procedures mainly to exclude tumors fractures or specific infections. The knowledge of pathophysiology means the knowledge of sympathetic and motoric efferences on one side and the functional examination of the anatomic structures on the other side.
