Abstract: Hans-Joachim Gerth Zusammenfassung Es wird eine Übersicht über die Poliomyelitis-Epidemiologie die Polioimpfstoffe sowie die Strategie und den derzeitigen Stand der WHO-Kampagne zur weltweiten Ausrottung der Poliomyelitis gegeben. Im zweiten Teil werden die Empfehlungen der Ständigen Impfkommission (STIKO) von 1994 zur Poliomyelitis-Impfung besprochen. Änderungen der Impfstrategie haben in den letzten Jahren in zahlreichen Entwicklungsländern unter alleiniger Verwendung der Sabin-Impfung zur Elimination der Polioviren geführt. In anderen Ländern fehlen aber z. Z. die finanziellen und organisatorischen Voraussetzungen für eine erfolgversprechende Ausrottungskampagne. Die Impfstrategie der STIKO stützt sich im wesentlichen auf die Poliolebendimpfung. Die Salk-Impfung wird nur für immundefiziente und HIV-positive Impflinge sowie Impflinge mit Kontakten zu immundefizienten oder HIV-Positiven empfohlen. Durch Kombination von Lebend- mit Totimpfung läßt sich eine sehr gute Immunität erzielen. Diese Möglicheit ist besonders dann interessant wenn andere Impfstrategien versagen. *) Überarbeitetes Manuskript zum Vortrag anläßlich des 30. Stuttgarter Kongresses für aktuelle Medizin der Ärztekammer Nordwürttemberg Abstract Poliomyelitis The epidemiology of poliomyelitis the poliovaccines and the present state of the WHO campaign for world-wide eradication of the polioviruses until the year 2000 are reviewed. In a second part the recommendations of the "Ständige Impfkommission (STIKO immunization task force)" from 1994 for Germany concerning poliovaccination are discussed. In contrast to the industrialized countries the oral vaccination of infants in the developing countries resulted in unsatisfactory protection rates. After changes in vaccination strategy including mass vaccination days the eradication campaign has succeeded in a considerable number of developing nations. However because of limited financial resources and organizational difficulties the year 2000 goal for global eradication of polioviruses will hardly be met. The vaccination strategy of the STIKO continues to rely essentially on the trivalent oral poliovirus vaccine. The enhancedpotency inactivated polio vaccine is recommended for immunodeficient and all HIV-positive vaccinees as well as vaccinees with immunodeficient or HIV-positive contacts. Combined use of oral live vaccine and inactivated poliovirus vaccine induces an excellent local and systemic immunity.
