Abstract: Since the early twenties, mistletoe preparations are used for parenteral treatment of cancer. In those days, the ingredients, the immunologic effects and the required dosage were not yet known. Founded on exact clinical observations, mistletoe therapy got its way until now as part of unconventional medicine and is nowdays examined even in some universities.However, the efforts to establish a valid dose schedule for mistletoe therapy were not successfull up to now. On account of our own clinical experience, especially in the application of high dose iv-infusions of mistletoe preparations, it seems very doubtful, if a rigid dose schedule will ever be possible or useful. Patients with advanced lung cancer react to mistletoe application in a quite different manner, concerning both immunmodulation and inhibition of tumor growth. At some patients a comparatively low dose results in a significant retardation of tumor growth, wherease at other patients, the same dose has only an immunmodulatory effect without alteration of cancer progression. A single case report demonstrates the reproduced inhibition of cancer growth by high dose iv-infusions of an aqueous nonfermented mistletoe extract. Sometimes a suppression of cellular immunoreactivity can be also observed. The choice of dose always depends on the patient's individual reaction.In addition, the possibility of the development of drug resistance to mistletoe will be discussed.
