Chirurgische Behandlung des Neuroblastoms

Hecker Kinderchirurgische Klinik im Dr. von Haunerschen Kinderspital der Universität München Zusammenfassung Die Grundprinzipien der Neuroblastom-Diagnostik und -Therapie werden dargelegt. Besonders die nicht seltene Beteiligung des Spinalkanals am Tumorgeschehen muß hierbei beachtet und kann durch ein Computertomogramm oder eventuell durch eine Kernspintomographie abgeklärt werden. Je nach Stadium der Tumorausbreitung ist eine operative oder kombiniert operativ-konservative (plus Chemotherapie Radiotherapie) Behandlung indiziert. Zweit- und Dritteingriffe sind gelegentlich notwendig. Die Ergebnisse der eigenen sowie der interdisziplinären Neuroblastom-Studie zeigen für die Stadien I und II eine 90%ige Heilungschance für das Stadium III in der Neuroblastom-Studie eine Überlebensrate von 72% und für das Stadium IV eine solche von lediglich 12%. Die Gesamtüberlebensquote beträgt 49%. Um die immer noch deprimierenden Überlebenszahlen der Stadien III und IV zu verbessern scheint das in Japan inaugurierte Neuroblastom-Screening erfolgversprechend. Surgical Treatment of Neuroblastoma: The basic principles of diagnosis and therapy of neuroblastoma are presented. Special regard has to be given to the not rarely found involvement of the spinal canal in the tumor's activity - a fact which can be ruled out by CT-scan or MRI. According to tumor staging operative or combined operative-conservative therapy (plus chemotherapy radiotherapy) is mandatory. Second and third look operations eventually may become necessary. Results of our own study and of an interdisciplinary study on neuroblastoma show for stage I and II a 90% chance of recovery; the neuroblastoma study reveals a survival rate of 72% for stage III and only 12% for stage IV of tumor distribution. In order to improve the still depressing figures of survival rates of the tumor stages III and IV the neuroblastoma screening method inaugurated in Japan seems to be promising. ab
