Intraoperative Bestrahlungstherapie (IORT) beim Magen- und Pankreaskarzinom

Krämling Chirurgische Klinik und Poliklinik der Ludwig-Maximilians-Universität München Klinikum Großhadern Zusammenfassung Die intraoperative Radiotherapie (IORT) erweitert die Möglichkeiten der chirurgisch sonst nur eingeschränkt erfolgreichen Behandlung des Magen- und Pankreaskarzinoms. Eine deutliche Verbesserung der Überlebensrate scheint nach japanischen Studien für bestimmte Stadien des Magenkarzinoms möglich. Die ersten Patienten mit Magenkarzinom konnten inzwischen auch in unserem Krankengut durch IORT behandelt werden. Während der letzten drei Jahre sind weiterhin 52 Patienten mit Pankreaskarzinom intraoperativ bestrahlt worden. Bei den potentiell kurativen R0-Resektionen wird eine Verbesserung der Überlebensrate angestrebt. Bei palliativ operierten Pankreaskarzinomen ist die lokale Tumorkontrolle das Behandlungsziel. Von 35 palliativ behandelten Patienten waren 85% nach der Operation weitgehend schmerzfrei - im Gegensatz zu 25% präoperativ. Eine lokale Tumorkontrolle ließ sich im Langzeitverlauf (13 Monate) bei 75% der Patienten erreichen. Diese bisherigen Erfahrungen zeigen eine mögliche Verbesserung der Ergebnisse der chirurgischen Therapie durch die IORT und ermutigen zur Weiterführung dieses Therapiekonzepts. Intraoperative Radiotherapy in the Treatment of Gastric and Pancreatic Cancer: Intraoperative radiation therapy (IORT) is a useful adjunct in the surgical treatment for cancer of the stomach and the pancreas. Japanese studies predict an improvement in survival rates for different stages of gastric cancer. We have now just begun to treat patients with gastric cancer by IORT. During the last three years 52 patients with pancreatic carcinoma were irradiated intraoperatively. In curative procedures we try to improve survival rates. In palliatively treated Gases local control of the tumor is the major aim of therapy. Out of 35 patients 85% were nearly completely free of pain - in contrast to 25% prior to operation. Local tumor control was successful in 75% of all patients as long-term follow-up demonstrates (13 months). These results show a possible improvement of the results of surgical therapy by the application of IORT. We are encouraged to continue this therapeutic concept. ab
