Diagnostik bei Enterovirus-Infektionen Enzymimmunoassay zum Nachweis von IgM-Antikörpern gegen Coxsackie-B-Viren

Stuttgart Zusammenfassung Es wird über einen Enzymimmunoassay zum Nachweis von IgM-Antikörpern (IgM-EIA) gegen Enteroviren (EV) berichtet der im Vergleich zur Komplementbindungsreaktion (KBR) eingesetzt wurde. Für beide Tests wurden die Coxsackie-Virustypen B1-B5 als Antigene verwendet. Bei 59 virologisch gesicherten EV-Infektionen (ECHO- Coxsackie-A- und Coxsackie-B-Virusinfektionen) konnte mit dem IgM-EIA in 56% und mit der KBR in 54% eine Diagnose gestellt werden. IgM- und KBR-positive Befunde korrelierten nur in 41 % der Fälle. Bei einem Vergleich beider Methoden an 891 Seren von Patienten mit Verdacht auf EV-Infektion konnten mit dem IgM-EIA in 46% und mit der KBR in 83% positive Ergebnisse erzielt werden. Der IgM-EIA ist nach vorliegenden Erfahrungen als alleiniger Test zur Diagnostik von EV-Infektionen nicht geeignet kann aber zusätzliche Informationen liefern wenn bei Verdacht auf EV-Infektion die KBR die Infektion nicht anzeigt. For both tests antigens of the coxsackie viruses B1-B5 were used. In 59 virologically confirmed EV infections (ECHO coxsackie A and coxsackie B virus infections) a serological diagnosis could be made by IgM-EIA in 56% and by CF test in 54%. IgM and CF test results correlated only in 41%. In 891 sera of patients with suspected EV infections positive results were obtained by IgM-EIA in 46% and by CF test in 83%. The results indicate that the IgM-EIA can supply additional informations to the CF test for diagnosis of EV infections but shouldn't be used as primary test. ab
