Thyreogene Akropachie Der besondere Fall

Abstract: Prof. Dr. H.-H. Goßmann Innere Abteilung Kreiskrankenhaus Siegen Zusammenfassung. Die thyreogene Akropachie ist eine seltene aber typische Komplikation der Basedow Krankheit. Sie wird bei 1% der Fälle - häufig nach jahrelangem Verlauf - beobachtet. Sie kann bei euthyreoter hypo- und hyperthyreoter Stoffwechsellage auftreten und ist meist Teil eines Syndroms bei dem außerdem eine endokrine Orbitopathie und ein prätibiales Myxödem besteht. Es wird über den Krankheitsverlauf einer Patientin mit diesen Erscheinungen berichtet und auf die Kriterien der Diagnose und die mögliche pathogenetische Rolle der Schilddrüsenantikörper bei der thyreogenen Akropachie eingegangen. Summary. Thyroid acropachy is a typical though rare complication of Graves' disease. It is seen in about 1 % patients often after a course of several years. It may occur while the patient is euthyroid hyper- or hypothyroid. In most cases it is part of a syndrome including exophthalmos and pretibial myxedema. We describe the course of a patient's disease and discuss the criteria of diagnosis as well as the possible pathogenetic role of antithyroid immunoglobulins in thyroid acropachy.
