Abstract: Aus der Klinik für Innere Medizin (Direktor: OMR Prof. Dr. sc. med. H. HALLER) Abteilung für Physiotherapie (Leiter: Doz. Dr. sc. med. A. LANGE) und der Klinik für Orthopädie (Direktor: MR Prof. Dr. sc. med. K.-J. SCHULZE) Abteilung für Physiotherapie (Leiter: Doz. Dr. sc. med. KLEDITZSCH) der Medizinischen Akademie "Carl Gustav Carus" Dresden Zusammenfassung Es wird eine kurze Übersicht zum Stand und zu den Entwicklungstendenzen der FESE als neue Rehabilitationsmethode für Hemiplegiker (Läsion des zentralen Neurons) sowie von Querschnittsgelähmten (Paraplegiker Quadriplegiker) infolge Rückenmarksläsionen mit FESE-Systemen für die obere und untere Extremität gegeben. Die Anwendungsmöglichkeiten als elektronische Orthesen und die noch zu wenig genützten Therapie-Trainingsgeräte werden erörtert. An Kontrollsystemen der FESE werden open-loop und closed-loop Systeme besprochen und Beispiele dazu angeführt. Sensoren-Technologien die Vor- und Nachteile von externen und implantierten on/off und proportional gesteuerten Systemen werden diskutiert. Im letzten Kapitel werden Ein- und Mehrkanal-Systeme efferente afferente und kombinierte Stimulations-Modelle und die verschiedenen Stimulationsmöglichkeiten zur Erzielung möglichst ermüdungsarmer proportionaler Muskelkontraktionen ("sequentiale" oder Karussel-Stimulation) dargelegt. Das sich rasch entwickelnde Gebiet der FESE verdient das Interesse der Physiotherapeuten Rehabilitationsmediziner und Bioingenieure gilt es doch damit die Lebensqualität hunderttausender motorisch behinderter Patienten entscheidend zu verbessern. Darüber hinaus hat es zu grundsätzlichen auch für andere elektrotherapeutische Anwendungsgebiete nützlichen Einsichten geführt. Summary A short review is given concerning the state and the developmental tendencies of the functional electric Stimulation of the extremities as a new rehabilitation method for hemiplegics (lesion of the central neuron) as well as for patients paralysed by a transverse lesion of the cord (paraplegics quadriplegics) due to spinal cord lesions with FESE-systems for the upper and lower extremity. The application possibilities as electronic or theses and the still too little used as therapy training devices are discussed. As to control systems of the FESE open loop and closed loop Systems are discussed and examples for this are cited. Sensor technologies the advantages and disadvantages of external and implanted an off and proportionally regulated systems are discussed. In the last chapter Single-channel and multi-channel Systems efferent afferent and combined Stimulation models and the various Stimulation possibilities for the achievement of proportional muscle contractions poor in fatigue ("sequential" and carousal stimulation respectively) are described. The quickly developing field of the FESE deserves the interest of the physiotherapists the rehabilitation specialists and bioengineers is it actually be taken into consideration for the decisive improvement of the life quality of hundreds of thousands of motorically handicapped patients. Moreover it has led to fundamental discernments being useful also for other electrotherapeutic fields of application. ___MH
