Zusammenfassung Stroke-Units sind Überwachungsstationen für Akutdiagnostik Sofortbehandlung und erste Rehabilitation von Schlaganfallpatienten. Indikationen für die Aufnahme in eine Stroke-Unit sind einerseits der Verdacht auf frischen Schlaganfall oder eine transitorische ischämische Attacke sofern die Symptome nicht länger als 24 Stunden bestehen sowie andererseits progrediente instabile Symptome. Eine wichtige Aufgabe der Stroke-Unit besteht in der Differenzialdiagnostik; hierdurch wird die bei Schlaganfall unaufschiebbare Behandlung festgelegt. Es gilt vor allem eine Progredienz der Symptome die bei einem Drittel der Patienten stattfindet und zu bleibenden Folgeschäden führt zu verhindern. Anhand von Fallbeispielen wird die Arbeit auf einer Stroke-Unit erläutert. Zur Verbesserung der Akutversorgung ist ein flächendeckendes Netz von regionalen und überregionalen Stroke-Units anzustreben. Summary Stroke units are monitoring centers providing for acute diagnostic work-up immediate treatment and initial rehabilitation measures in patients suffering a stroke. Indications for admission to a stroke unit are firstly a suspected fresh stroke or a transient ischemic attack provided the symptoms are not older than 24 hours and secondly progressive instable symptoms. A major aspect of the stroke unit is the establishment of a differential diagnosis which provides the basis for establishing the urgent treatment of an apoplectic attack. The goal is to prevent the progression of symptoms which occurs in onethird of patients and leads to permanent sequelae. Using a number of cases the activities of a stroke unit are described.
