Wie groß ist der Bedarf für ambulante Formen der Rehabilitation im Bereich der Orthopädie? - Ergebnisse eines Mehrperspektivenansatzes

Wolfgang Bürger Abteilung für Medizinische Psychologie am Universitätskrankenhaus Eppendorf Hamburg Zusammenfassung: Trotz Forderungen nach Ausbau eines ambulanten rehabilitativen Versorgungssystems verläuft die diesbezügliche Entwicklung in der Bundesrepublik Deutschland bislang noch zögerlich. In diesem Zusammenhang bestand bislang ein erhebliches Informationsdefizit über den Bedarf für solche Angebote. Im Rahmen eines komplexen Studienansatzes wurde der Bedarf an ambulanten medizinisch-rehabilitativen Maßnahmen im Indikationsbereich Orthopädie aus der Perspektive von Versicherten Gutachtern eines sozialmedizinischen Dienstes Ärzten einer Rehaklinik sowie niedergelassenen Ärzten untersucht. Die Daten zeigen übereinstimmend einen substantiellen Bedarf an ambulanter Rehabilitation der bei etwa einem Drittel der rehabilitationsbedürftigen Versicherten und damit deutlich über den bisher realisierten Versorgungsanteilen liegt. Die Ergebnisse weisen aber auch darauf hin daß die befragten Ärzte in ihren Schätzungen unsicher sind und es aus den unterschiedlichen Perspektiven (Versicherte verschiedene Ärztegruppen) bezogen auf einen konkreten Versicherten sehr häufig zu unterschiedlichen Präferenzen hinsichtlich der Form der rehabilitativen Versorgung kommt. Die Befunde sprechen für eine verbesserte Information von Versicherten und zuweisenden Ärzten über das neue Angebot der ambulanten Rehabilitation für die Entwicklung von differentiellen Indikationskriterien für ambulante und stationäre Rehabilitation und die Entwicklung von klaren Verfahrensregelungen die möglichst zwischen allen Kostenträgern abgestimmt sind. What is the Demand for Orthopedic Outpatient Rehabilitation? Results of a Multiple Perspective Concept: Despite the necessity to expand the outpatient rehabilitation system in Germany the development up to now must be described as quite reluctant. As yet there is a considerable lack of information about the demand for such care in Germany. On the basis of a complex study design insurants applicants for medical rehabilitation physicians in a rehabilitation hospital and general practitioners were asked how they estimated the demand for outpatient orthopedic rehabilitation care. Resulting data unanimously show a substantial demand in roughly one third of insurants who desire rehabilitation clearly exceeding the already realized outpatient care. The investigation also illustrated that physicians often are uncertain as to their decisions and that depending on the respective point of view (insurants or different groups of physicians) there are different preferences as to the kind of rehabilitative care for the benefit of one specific patient. The results stress not only the fact that there is a demand for information of insurants and their referring physicians regarding the new outpatient rehabilitation offer but also reveal the demand for a development of differential indication criteria for outpatient and inpatient rehabilitation as well as for definite procedural regulations which should be harmonized among all insurers.
