Die ambulant erworbene Pneumonie stellt auch in der Ära neuer antibiotischer Behandlungsmöglichkeiten in der Praxis eine ernstzunehmende Erkrankung dar. Es kann jedoch bei über 80% der Erkrankten eine ambulante Therapie erfolgen wodurch erhebliche sozio-ökonomische Kosten eingespart werden. Aufgrund der geringen Spezifität der syndromorientierten Einteilung und der radiologischen Befunde wird heute eine klinische Beurteilung nach Risikofaktoren bevorzugt die bereits wertvolle Hinweise auf das zugrundeliegende wahrscheinlichste Erregerspektrum zuläßt und einen Leitpfad für die "kalkulierte"Antibiotika-Behandlung darstellt. Abstract Community-acquired pneumonia in the adult. Current aspects of differential therapy in practice: Community-acquired pneumonia remains a common and serious illness despite the availability of potent new antimicrobials. More than 80% of the patients can be treated on an out-patient basis thereby saving money. Because of the nonspecificity of clinical and radiographic findings and the limitations of diagnostic testing for identifying an etiologic pathogen most initial therapy is necessarily empirical. Two major variables that influence the spectrum of etiologic agents and the initial approach to therapy are: severity of illness at initial presentation and presence of either coexisting illness or advanced age.
