Effect of glucucorticoids (GCS) and non-steroidal anti-inflammatory drugs (NSAID) on gastro-intestinal tract mucous in patients with rheumatic diseases

Journal/Book: Z Rheumatol 1998; 57 Suppl. 1: 28 (F 11). 1998;

Abstract: Institute of Rheumatology of RAMS Moscow The problem of risk of ulceration and erosions of the mucous in upper regions of gastrointestinal tract (GIT) requires further investigation. Prevalence of gastropathies in patients taking NSAID (Group 1 n = 726) NSAID and GCS (Group 2 n = 238) and only GCS (Group 3 n = 122) was studied. Group 1 included patients with RA OA and AS Group 2 - patients with RA and SLE Group 3 - patients with SLE and polymyositis. Patients were included in the study in cunsecutive order. There were no statistically reliable differences as to sex GIT ulceration in history smoking and cytotoxic drugs taking between the groups. Prevalence of gastropathies revealed by endoscopy according to the age is demonstrated in Table. Groups Age (years) n Multiple erosions Ulcers < 30 97 7 (7.2 %) 14 (14.4 %) 1 30 - 50 311 41 (13.2 %) 46 (14.8 %) 51 - 65 227 26 (11.4 %) 36 (15.9 %) > 65 81 7 (8.6 %) 22 (27.2 %) < 30 36 0 2 (5.5 %) 2 30 - 50 95 7 (7.4 %) 9 (9.5 %) 51 - 65 75 5 (6.7 %) 18 (2.4 %) > 65 26 1 (3.8 %) 2 (7.7 %) < 30 40 2 (5 %) 0 3 30 - 50 62 1 (1.6 %) 1 (1.6 %) 51 - 65 14 0 0 > 65 4 1 (25 %) 0 Obtained data demonstrate that gastropathies during GCS treatment occur significantly more rare (p = 0.001) as compared with those during NSAID treatment whereas taking NSAID with GCS does not increase the risk of gastropathies as compared with taking only NSAID. le
