Abstract: There are many different reasons for osteopathy in kidney patients besides the lack of vitamin D and the secundary hyperparathyroidism. Patients with adynamic bone disease (ABD) with both low alkaline phosphatase and parathyroid hormon are seen more often. Bone disturbances may commence in the beginning of a kidney disease with proteinuria. Later on a variety of risk factors, e.g. metabolic acidosis, heparin, dialysate calcium, vitamin D medication, magnes dialysate calcium and vitamin-D-therapy. In osteopenia additional low dose fluoride may be indicated. In the future vitamin-D-analogues with less risk of hypercalcemia will be available.
