Wetteraner Urologische Klinik Universitätsklinikum Freiburg Chancen und Risiken Mit dem Phosphodiesterase-V Inhibitor Sildenafil (Viagra(r)) steht zum ersten Mal eine sowohl bei organisch wie psychisch bedingten erektilen Dysfunktionen wirksame Medikation zur Verfügung. In diesem Beitrag werden zuerst die Ätiologien der erektilen Dysfunktion und mögliche orale pharmakologische Behandlungs-möglichkeiten dargestellt. Ausgehend von gegenwärtigen Konzepten von Sexualität werden mögliche soziokulturelle Konsequenzen der Medikamenteneinführung aufgezeigt und das Mißbrauchspotential betrachtet. Gesellschaftsverändernde Grundlagen einer Verschiebung von der Pharmakotherapie zu Pharmakoprävention durch sog. Lifestyle-Drogen werden erörtert. Abstract Effective oral pharmacotherapy of erectile dysfunction - chances and risks: The new phosphodiesterase V inhibitor sildenafil (Viagra(r)) is the first pharmacological agent that can be successfully used in the treatment of organic as well as psychogenic erectile dysfunction. In this article we summarize the different etiological factors that contribute to erectile dysfunction and discuss the contesting oral medications. Using current concepts of sexuality we show the sociocultural impact of the new treatment modality. The high Potential for abuse is highlighted. We demonstrate the changes in society that will be induced by Viagra(r) and the cultural changes that led to the introduction of a new class of drugs. The change from pharmacotherapy to more or less pharmacoprevention and "lifestyle-drugs" is discussed.
