Grundsätze und Variationsmöglichkeiten in der antihypertensiven Kombinationstherapie

Basics and variations in the combination therapy of hypertension Zusammenfassung Die medikamentöse Monotherapie der Hypertonie führt bei durchschnittlich der Hälfte aller Patienten zum Erfolg. Das Auftreten relevanter Nebenwirkungen bedingt in der Regel einen erneuten Behandlungsversuch mit einem Antihypertensivum einer anderen Substanzklasse (= sequentielle Monotherapie). Wird dagegen bei guter Verträglichkeit keine Normotonie erreicht sollte der Arzt zwei Hochdruckmedikamente unterschiedlicher Angriffspunkte miteinander kombinieren. Ähnlich wie bei der Monotherapie unerläßlich ist auch hierbei ein individuell abgestimmtes Vorgehen auf den Patienten - seine Kreislaufsituation vorhandene Risikofaktoren oder Begleitkrankheiten - bestimmend für Erfolg bzw. Mißerfolg. Im folgenden werden sinnvolle und klinisch bewährte Kombinationsmöglichkeiten mit den "first-line"-Präparategruppen beschrieben. Unter den Aspekten Ökonomie Pragmatismus und Patientencompliance wird u. a. auf bewährte und neue Fixkombinationspräparate gesondert eingegangen. Abstract The drug monotherapy of hypertension is successful in about half of the patients. The occurrence of relevant sideeffects usually leads to a new treatment with an antihypertensive drug of another substance category (= sequential monotherapy). However if normotension is not achieved despite good tolerance the physician should combine two antihypertensive drugs with different modes of action. Similar to the monotherapy a treatment procedure individually balanced to the patient- situation of the circulation risk factors or coexisting diseases - determines success or failure. In the following useful and clinically reliable combination options with first-line substance categories are described. Under the aspects of economy pragmatism and patient compliance among other things it is dealt with reliable as well as new fixed combination preparations.
