Antibiotikatherapie in der Praxis

Abstract: Dietrich Höffler Städtische Kliniken Darmstadt Medizinische Klinik 111 Darmstadt Zusammenfassung Die antibiotische Therapie ist nur sinnvoll beim Vorliegen eines bakteriellen Infektes. Es werden Möglichkeiten dargelegt wie eine virale Infektion - in der Praxis der häufigste Grund von Fieber - von einer bakteriellen zu trennen ist. Hierbei ist u. a. das CRP hilfreich das nur bei einer bakteriellen Infektion deutlich ansteigt. Sein Abfall zeigt an daß die Therapie erfolgreich ist und das Antibiotikum abgesetzt werden kann. Ist der Sitz der Infektion (z.B. Harnwege ) bekannt kann auf die Keime und deren Empfindlichkeit gegen Antibiotika rückgeschlossen werden. Mit dieser ".kalkulierten Therapie" kann der Einsatz "falscher" Antibiotika minimiert werden. Die in der Praxis verwendbaren Antibiotika werden unter Berücksichtigung von Wirksamkeit Nebenwirkungen und Preis in ihren Indikationen und Kontraindikationen gegeneinander abgewogen. Abstract Antibiotic therapy in the general practise Antibiotic therapy only makes sense if a bacterial infection exists. Strategies are discussed how viral infections - the most important reason of fever in general practise - can be differed from bacterial infections. Of special importance is e.g. CRP which only rises in case of a bacterial infection. Its decline shows that therapy is successful and that the antibiotic therapy can be discontinued. If the location of the infection is known (e.g. urinary tract) one can expect certain microorganisms and their sensitivity to antibiotics. With this Kind of "calculated therapy" the use of "wrong" antibiotics can be minimized. The antibiotics which can be employed in general practise are discussed by terms of effectiveness side effects and price in relation to their indications and contraindications. wt
