Milzruptur bei infektiöser Mononukleose

Schmidt) Zusammenfassung Bei einem 22jährigen Patienten mit infektiöser Mononukleose der in der Anamnese eine Prellung der linken Thoraxwand angab kam es zu einer Schocksymptomatik unter dem Bild eines akuten Abdomens. Es wurde der Verdacht auf eine Milzruptur bei infektiöser Mononukleose erhoben der sonografisch untermauert und durch die Splenektomie bestätigt wurde. Die Darstellung dieser seltenen Komplikation - Folge der erhöhten Vulnerabilität der Milzkapsel bei infektiöser Mononukleose - soll an die Möglichkeit der Milzruptur erinnern. Die sofortige Splenektomie gilt als sicheres und kompromißloses Vorgehen. Abstract Spleen rupture in a case of infectious mononucleosis A 22 year old male with infectious mononucleosis who stated a history of trauma to his left thoracic wall developed an acute abdomen with symptoms of shock. A ruptured spleen was suspected. This working diagnosis which was supported by ultrasound led to a splenectomy the initial diagnosis. The case report of this rare complication - due to the increased vulnerability of the spleen capsula in infectious mononucleosis - is highlighting the possible danger of splenic rupture.
