Abstract: DSM-IV acknowledges, to a much greater degree than previous editions, the importance of culture in shaping the symptoms, expression, and course of major mental illness. Many challenges remain, however, especially for the clinician attempting to extend the diagnostic criteria to other cultural populations. This article systematically reviews key assumptions underpinning the DSM-IV approach to major depression and, through the use of two case examples as well as the available literature, highlights a number of issues that must be taken into account in meeting these challenges.
