Abstract: Aside from the classic side effects of antibiotics (e. g. allergies) consequences resp. secondary illnesses occasionally occur, in which damage of the gut flora play" a central role. Disorders of the gut flora induced by antibiotics not only frequently lead to acute diarrhoea, they also cause longterm "functional" bowel problems, e.g. diarrhoea and troublesome flatulence as well as constipation and food indigestibility. Antibiotics can create niches for fungal infections within the complicated microecological system of the gut and, furthermore, they may by means of selection lead to a sepsis due to highly padiogenic microorganisms from the gut lumen in immuno-comprornised patients. Especially in the case of biliary eliminated antibiotics the damage caused is the more important the more ß-lactamase-resistant the preparation is as well as the more effective it is against gram-negative or anaerobic bacteria. A normal bowel flora usually prevent" a massive colonization of the gut by pathogenic bacteria solely by its presence (so-called colonization resistance).orally applied preparations also lead to a disturbance of the intestinal flora, which depends on the individual range of action of the drug as well as on the resorption, ;peed. In general, one has to consider carefully, whether the use of antibiotics is indicated in a certain case or not. In case a long-term therapy with antibiotics is unavoidable the recultivation of the gut flora should be part of the therapeutical concept.
