Abstract: In the past decade, the really new thing with respect to a prevention of hyperuricaemia and gout was not so much the change of the kind and extent of the dietetic intervention but rather a reclassification of the persons, which are the target of appropriate dietetic recommendations. As a result of an early therapeutic treatment when hyperuricaemia was recognized, a manifest gout disappeared nearly totally or its typical clinical picture was disguised despite an unbroken increase in the frequency of hyperuricaemia among the population. Therefore it turned out to makes sense to talk about "potential" gout in cases which would result in a manifest gout without treatment. Such patients need special attention because they can show similar to patients with manifestes gout a number of concomitant diseases with an atherogenic potential, as for example disturbances of the fat and carbohydrate metabolism, high blood pressure, changes of the liver and the kidneys. The way of life of the people concerned has to be organized in such a way, that they can manage with the smallest dosage of drugs. The intake of a "reasonable" diet as a standard diet is recommended, as it is used for prophylaxis and treatment of atherosklerosis and diabetes mellitus, of disturbances of the lipometabolism and high blood pressure. Food rich in purine (innards) must be avoided. Analogous to a ”reasonable" diet it is required in the case of extreme obesity to reach a "reasonable" body weight. Health education is especially important from the point of view of preventive medicine when hyperuricaemia and gout are present.
