Abstract: The keywords which are important for a proof of action, i.e. objectivity, causality and universality are explained in detail in the present work. The controlled clinical trial with a respective biometric planning and evaluation most likely does justice to theses terms. Considerable doubts have been and are still expressed about carrying out such studies. The doubts mainly have an emotional and ethical basis. Also the methodical objection can be raised, that by the specific method of study, an artificial and experimental situation is created, which has nothing in common with the conditions of application in practice, so that the results of the study can hardly be transferred to practice. Alternatively, remedies, which have been mainly handed down and used for a long time, can be used to assess the efficacy by a systematic observation of the successes or failures during the practical application. Every general practitioner knows, that with some patients a good effect of a certain drug can be expected (responder) and with other patients a bad or no effect (non-responder). Approaches for models are presented, which take into consideration these different individual chances. What is especially important, is the work out prognostic criteria for an individual chance of efficacy.
