Abstract: The parameters that influence the pharmacokinetics of a medicament are absorption, distribution, metabolization and, finally, excretion. The typical organic and metabolic changes that accompany old age may be reflected at each of these stages. Frequent achlorhydria and reduced stomach peristalsis lead to poor absorption of, for example, substances containing folic acid, iron, calcium and penicillin V On the whole, however, the resulting changes tend to intensify the medicinal effects, among them transport mechanisms that lead to increased free effective concentrations of medicaments in the presence of reduced albumin concentrations in old age. Renal and hepatic drug elimination is also generally reduced. This has been demonstrated for many different medicines, in particular those indicated most frequently such as digoxin, diuretics, many antibiotics, tricyclical benzodiazepines, etc.
