L-Tryptophan-assoziiertes chronisches Eosinophilie-Myalgie-Syndrom behandelt mit Ciclosporin

Abstract: Prof. Dr. med. R. Dreher Klinik für Rheumakranke Bad Kreuznach Zusammenfassung: Eine 52 Jahre alte Frau entwickelt 2 Wochen nach Einnahme von 130 g L-Tryptophan zunächst im Anschluß an eine Unterarmvenenthrombose ein akutes Eosinophilie-Myalgie-Syndrom mit möglicher flüchtiger kardialer Beteiligung bei intermittierender Sinustachykardie. Im Anschluß entsteht ein schweres chronisches (follow-up 15 Monate) Krankheitsbild mit ausgeprägtem diffusem Skleroderma und schwerer sensomotorischer Polyneuropathie. Histologisch zeigt sich eine mononukleäre Infiltration der Faszie und interstitielle Myositis eine anfängliche Bluteosinophilie (900/ul) ist im weiteren Krankheitsverlauf nicht mehr nachweisbar. Als Ausdruck einer Störung des Tryptophanstoffwechsels ist der Kynureninplasmaspiegel stark erhöht (4000 pmol/ ml) der Kollagenstoffwechsel gesteigert (ProkollagenTyp-III-Peptid 0 927 U/ml). Nach Nichtansprechen einer Therapie mit Azathioprin (100 mg/die) und Prednisolon (40-60 mg/die) bildet sich das Skleroderma unter Ciclosporin (150 mg/die) vollständig zurück die Polyneurophatie persistiert es bestehen weiterhin leichtere Myalgien. Summary: After 2 weeks of ingestion of 130 g L-Tryptophan a 52 year old female developes an Eosinophilia Myalgia Syndrome with acute onset of deep venous thrombosis of forearm and possible initial cardiac manifestation featuring intermittent sinustachykardia. This is followed by a severe chronic disease (follow-up 15 months) with diffuse scleroderma and sensomotoric polyneuropathia. The deep muscle biopsy-specimen shows mononuclear infiltration of fascia and interstitial myositis with rare eosinophils. A blood eosinophilia (900/ul) occurs only in the initial acute onset of the illness. Plasma level of Kynurenine is significantly high (4000 pmol/ml) collagenneosynthesis is activated (Procollagen type III peptid 0 927 U/mI). No significant clinical improvement was seen with Acathioprine (100 mg/d) and Prednisolon (40-60 mg/d) after treatment with Ciclosporin scleroderma regresses completely polyneuropathy is persisting.
