Mobile Ergotherapie für Rheumakranke: Entwicklung des Dienstes Inanspruchnahme und Versorgung mit Hilfsmitteln und Schienen

Abstract: Dipl. Soz.-Wiss. B. Grocholski-Plescher Rheuma-Liga Schleswig-Holstein Kiel Zusammenfassung: Untersucht werden die Entwicklung und Inanspruchnahme eines landesweiten durch die Rheuma-Liga Schleswig-Holstein neu aufgebauten ergotherapeutischen Dienstes zur Hausversorgung von Rheumakranken. Vom 1.7.86-31.6.91 wurden 1120 Verordnungen von 293 Ärzten aus 86 Orten ausgestellt. Es konnten damit 528 Patienten in einem Umfang von 6778 Behandlungseinheiten versorgt werden. Die Entwicklung des Dienstes zeigt eine langsame aber stetig wachsende flächendeckende Akzeptanz durch die verordnenden Ärzte und eine entsprechend steigende Inanspruchnahme die vermehrt auch nicht-rheumatologische Diagnosen umfaßt. Die Hilfsmittel- (in 55 % aller Verordnungen) und die Schienenversorgung (in 26 % aller Verordnungen) werden gesondert erläutert da Beratungsleistungen und praktischen Hilfestellungen - insbesondere im Zusammenhang mit alltäglich nützlichen Hilfsmitteln - eine große Bedeutung zukommen. Summary: The development and utilization of occupational therapy as a homecare service for patients with rheumatic diseases is examined. This new service was implemented in Schleswig-Holstein by local branch of the German League against Rheumatism an organization devoted to the promotion of help and self-help for patients. The data for this study are based an standardized documentation's within a comprehensive evaluation of model-intervention and also accounting records. From July 1 1986 until June 31 1991 there were 1120 prescriptions issued by 293 physicians at 86 locations. 529 patients could be served with a total amount of 6778 time-units of care. The development of the service shows a slowly but steady and regionwide growing acceptance by the practitioners and accordingly an increasing utilization associated with an increasing inclusion of nonrheumatic diagnoses. The supplies of support devices (included in 55 % of all prescriptions) and braces (included in 26 % of all prescriptions) are described separately since patient counseling and assistance - particulary in connection with support devices and aids for everyday life - are of great importance.
