, ,Abstract: Endogenous lectins assume several control functions in the human immunological system. Certain plant lectins could interfere in process chains which are used by endogenous lectins. The basis for every lectin effect is the integrity of the sugar binding. With a clinically applied mistletoe extract (Iscador®) the ß-galactoside-specific lectin has an immunomodulating effect. In the mediation of this effect the increased secretion of the tumor necrosis factor-*, interleukin 1 and interleukin 6 which has been induced by the lectin, plays an important role. The stimulation of the cytokine secretion depends on the binding of the lectin to cellular glycoconjugates. The immunomodulation in humans caused by the mistletoe extract can be compared with the immunomodulation caused by an isolated ß-galactoside-specific mistletoe lectin. This similarity only refers to the sugar binding amount, which is determined by an optimized ELLA procedure. The optimum dosage for this lectin activity in the extract turned out to be 1 ng/kg of the weight of the patient. The consideration of the sugar binding lectin content can lead in the mistletoe therapy to an optimized immunomodulation, which in turn showed significant antitumoral effects in a first clinical study. These antitumoral effects were only the result of mistletoe preparations, which cause an immunomodulation comparable to pure lectin.
