Zur Wirksamkeit verschiedener krankengymnastischer Programme bei erwachsenen Myopathiekranken*

Abstract: The goal of the study was the evaluation of the effect of static and dynamic methods of physiotherapy an adults with inherited forms of myopathies. We report the results of 245 in-hospital treatments of 110 patients with progressive muscular dystrophy, 39 pat. with spinal and 26 pat. with neural muscular atrophy. Specifically designed functional tests were carried out at the beginning and end of each in-hospital treatment (average duration 47 days) and a comparison was made. Change of plus or minus 20 % were considered an improvement or deterioration. Six different regimes of physiotherapy were compared with one another. Each was performed five times a week and included isometric exercises of 30 to 60 minutes duration as well as dynamic exercises performed conventionally or either in a kinotherapeutic bath or in a “Schlingentisch” to eliminate the effects of gravity. These therapies were also performed with a nonstandardized group of patients.Results: Only certain functional tests showed themselves appropriate to measure changes. The sensibility of the measurement of muscular strength or of ADL-scales was insufficient. All regimes of physiotherapy achieved some improvements in most patients. Isometric exercises of increasing duration improved capacity of locomotion most often (68-73 %). Dynamic exercises improved more complex functions like changes of Position, climbing stairs etc. (43-60 %) and functions of the upper extremities (50-62 %). Deterioriation of single functions was rarely seen.Conclusions: Patients with myopathies treated with physiotherapy received benefits including improved functions and increased capability to cope with daily life. Depending an patients individual condition therapy should consist of static and dynamic exercises. Patients prefered dynamic exercises, the acceptance of static exercises alone was lower.
