Somatische Therapie chronifiziert depressiver Patienten

Möller Direktor der Psychiatrischen Klinik und Poliklinik der Universität Bonn Zusammenfassung Die Behandlung chronifiziert depressiver Patienten macht besondere Anstrengungen erforderlich. Als erster Schritt sind Höherdosierung parenterale Applikation oder Wechsel des Antidepressivums zu empfehlen. Falls auf diese Weise eine ausreichende Besserung nicht erreicht werden kann sind folgende Kombinationsbehandlungen sinnvoll: Kombination eines Antidepressivums mit Lithium Kombination eines Antidepressivums mit Schilddrüsenhormon Kombination ein Antidepressivums mit MAO-Hemmern. Als dritter Schritt sollte die Elektrokrampftherapie in Betracht gezogen werden. Bei wahnhaften Depressionen ist die Kombination eines Antidepressivums mit einem Neuroleptikum notwendig. Somatic Treatment of Chronically Depressed Patients: Treatment of chronically depressed patients makes special efforts necessary. As a first step higher dosage parenteral application of the antidepressant or change from one mode of action to another should be tried. If this is not successful as a second step the following combinations are indicated: combination of lithium with an antidepressant combination of thyroid hormone with an antidepressant and the combination of an MAO-inhibitor with an antidepressant. As a third step electric convulsive therapy should be considered. In case of delusional depression the combination of the antidepressant with a neuroleptic is necessary. ab
