Zwerchfellrupturen beim Polytrauma und Dringlichkeit ihrer Versorgung

Stiegler Chirurgische Klinik und Poliklinik der Universität München Klinikum Großhadern Zusammenfassung Zwerchfellrupturen nach isoliertem Trauma sind seltene Komplikationen; nach einem Polytrauma jedoch muß häufiger mit ihnen gerechnet werden. In unserem Krankengut fanden sich Zwerchfellrupturen bei 40 von 935 (4 4%) der Patienten mit Polytrauma (Zeitraum 1. 1. 1978-1. 1. 1989). Sie traten in nahezu 80% linksseitig auf; trotz früher Versorgung lag bereits bei 45% ein Prolaps intestinaler Organe vor. Der Operationszeitpunkt war von den Begleitverletzungen abhängig. So standen in 78% schwere blutende abdominelle Läsionen im Vordergrund die zur sofortigen Laparotomie zwangen. Bei sorgfältiger operativer Exploration sollte eine Zwerchfellruptur nicht zu übersehen sein. Besteht Zeit zur kompletten Diagnostik so sollte nach der Diagnose dringlich eine Zwerchfellnaht erfolgen um kardiorespiratorische Komplikationen und Inkarzeration zu vermeiden. Diaphragmatic Ruptures in Patients with Polytrauma and Urgency of Treatment: Diaphragmatic ruptures are rare complications after an (isolated) trauma but they more often appear in the polytraumatized patient. 4 4% (n = 40) of our 935 cases with polytrauma had diaphragmatic ruptures (1.1. 1978-1. 1. 1989). Almost 80% of these happened on the left side in 45% of all cases there was a prolapse of the intestines in spite of early treatment. The best time for surgical intervention depended on the accompanying lesions. In 78% of all cases severe abdominal bleeding was predominant and necessitated immediate laparotomy. Careful surgical exploration should exclude the possibility of overlooking a diaphragmatic rupture. If there is time provided for a complete check-up the suture of the diaphragm should immediately follow the diagnosis in order to prevent cardiorespiratory complications and incarceration. ab
