Endokrinologie und Stoffwechselkrankheiten Innere Medizin I der Universität Heidelberg Zusammenfassung Maligne Tumoren von Mamma Prostata Schilddrüse und Bronchialsystem metastasieren präferentiell in die Knochen. Knochendestruktion ist auch eine Folge hämatologischer Systemerkrankungen wie Plasmozytom oder Leukämie. Schmerz entsteht durch Knochenbruch mit Weichteilzerreißung Gelenkfehlstellung und Periostzerrung; er begleitet osteolytische Metastasierung auch ohne manifeste Verformung der Knochenkontur. Schwere neurologische Komplikationen mit Schmerz und Paresen drohen vor allem bei Wirbelsäulenbefall. Kalzitonin wirkt bei etwa 50% der betroffenen Patienten schmerzlindernd. Der Einsatz von Bisphosphonaten bremst die Progression der osteolytischen Metastasierung und ist so indirekt wirksam. Im übrigen gelten die Regeln der allgemeinen Onkologie bei der Schmerzbehandlung. Bone destruction also develops in haematological systemic diseases such as plasmacytoma or leukaemia. Pain originates from bone fracture with soft-tissue lacerations defective joint position and periosteal strain; it accompanies osteolytic formation of metastases even without manifest deformity of the bony contour. Severe neurological complications with pain and pareses are particularly imminent in the event of spinal involvement. Calcitonin alleviates pain in about 50% of the patients affected; the use of bisphosphonates reduces the progression of osteolytic filial spread and is thus indirectly effective.
