Abstract: After a comparative representation of differances, advantages and disadvantages of general and regional anaesthesia we scrutinized special indications of regional anaesthesia. In comparison with general regional anaesthesia is said to be safer and of less risk. On the otherhand it is loaded with more anxiety, fear, and stress.Therefore the preoperative treatment of the patient with anxiolytica is justified. Most commonly used are benzodiazepine compounds and neuroleptica. Phytopharmaca are practically not in use, as the saleble compounds do not show a sufficient immediate effect.By means of a pilote study we found out that 60 mg Kawalactones were the adaequate dosis for regional anaesthesia. The same dosis was given to the patient in the evening the night before and the next morning one hour before the operation.A double-blind randomised study with the same dosis showed three remarkable results. By dividing the patients into four groups with a planned operation duration of a) less than40 min b) 40-80 min c) 80-120 min and d) more than 120 min verum and placebo turned out to have nearly the same good results in the first group. In the forth group the results were nearly equally bad. Whereas in the both middle groups Kavosporal proved to be of doubtless advantage compared to the placebo.From our point of view phytopharmaca of an adaequate dosis of kawa-kawa extract effect a good and sufficient anxiolysis for middle lasting surgery.
