Abstract: Psoriasis is the commonest skin disorder in the world. Despite intensive research and the development of new treatments, the exact pathogenesis of this illness has not been established and therefore all efforts to find a successful treatment with few side-effects have failed. External therapy such as ointments, baths and the combination of photo- and chemotherapy can only lead to a temporary, cosmetic improvement of the skin. Recent research has however revealed, that we are not dealing with a skin disease at all, but with a complex metabolic disorder, in which a failure in fumaric acid synthesis appears to play an important role. Our aim should therefore be to correct this defect or at least to supply the body with the necessary fumaric acid.In over 700 treated patients,, fumaric acid substitution and strict adherence to a diet, led in over 90% of patients within a few weeks or months to a complete clearing of the skin lesions. Side-effects such as epigastric discomfort, heart burn, nausea, vomiting, diarrhoea, skin flushes and itching of the skin. very rarely led to a discontinuation of the treatment.Of all the treatments for Psoriasis available at the present time, fumaric acid Substitution and diet is certainly the most effective and has the least side-effects.
