Abstract: An 18 year old boy was admitted to hospital because of muscle weakness.Laboratory studies revealed hypokalemic alkalosis, aldosteronism, andincreased plasma renin activity. The patient was normotensive and showedreduced vascular sensitivity to exogenous angiotensin II. Renal biopsyshowed hyperplasia and hypertrophy of the juxtaglomerular apparatus.These findings were consistent with a diagnosis of Bartter's syndrome.Further, the patient had a tendency towards erythrocytosis and developeda significant rise in plasma erythropoietin activity.
